    How to Order CPAP\/BiPAP Supplies<\/h1>\n

    If you are a current patient, you are on CPAP\/BiPAP, and you have been seen in our office recently, you may request PAP supplies by telephone. Please call (615) 893-4896 for the Sleep Center of Murfreesboro. You will be asked to provide the following information:<\/p>\n

    • Full name<\/li>\n
    • Date of birth<\/li>\n
    • PAP supplies needed<\/li>\n
    • Name of prescribing physician at our facility<\/li>\n
    • Name of your durable medical equipment<\/li>\n<\/ul>\n

      PLEASE NOTE: Your insurance carrier may require that a follow-up appointment be conducted to ensure treatment efficacy and eligibility for before ordering PAP supplies.<\/p>\n
